How does infections occur if Germ Theory is invalid?

For example: It is commonly said in western medicine that if a wound is unwashed or treated poorly it can get infected and can lead to higher consequences such as amputation, and this has been observed, no?

Its probably not an infection in the typical sense but toxins entering the wound which makes it react
